  • Full name, residential and mailing address, phone number, age, date of birth, race, social security number, birthplace, relationship of parties (if any), designated surname, number of prior marriages and how they were dissolved, parents' names and birthplaces, and the date and place of the intended marriage.

  • Without Premarital Education Certificate: $76 (cash only).
  • With Premarital Education Certificate: $36 (cash only, with proof of completing six hours of premarital counseling within the last 12 months).

  • The officiant will sign the license after the ceremony and return it to the court.
  • The court will issue a certified copy of the marriage license and application for name changes on legal documents.
